I also haven't found a defect in the code.
It sounds to me that you could change the covers order once upon a time and the old thumb is just cached?
Try to clean up /Portable/Temp/Thumbs/ , /Portable/Temp/AlbumImages/ , /Portable/Temp/ImageCache/
Does it resolves the issue ?
Another possible cause could be that the album tracks have inconsistent artwork and the album thumb was created based on first file of the album (that can have the incorrect cover assigned) ?

Looks like you are right in your second assumption.
First track have reverse order of the images, So once I select all track in album -> Properties (Mass Edit) -> Artwork it shows different order than on a track (eg. order from first track) So only way to correctly show Album art is to retag the whole Album.

I guess this is something we should look into for 5.1? |

I do not think that taking the album cover as first cover of the first track is a bug. |

You are right it is not a bug, but this is another case of missing dedicated Album Art against track Images that users demand.
Commonly seen in compilations where there Album Art is commonly wrong and is better to have ability to add album image.
Left open till 5.1 as with new UI it is one of common issues requested.
